I was at the end of my rope

I was at the end of my rope. I've lost and gained weight for 30+ years. I tried to see my doc and my appointment was 2 months out. I did some research and found Form. I got an appointment much quicker and began a very satisfying journey. I pay for Ozempic out of pocket, but have been successful on a very low dose. I'm down 35# since January and about at my goal. I have a great doctor and nutritionist at Form. Any questions I have are quickly answered. I highly recommend this service.

Charged immediately before any services were provided

I signed up with Form, after it having been on a list provided by my insurance company. I was told there would be an undefined wait to be able to access any services, which I understood. What I didn’t expect was for my credit card to be charged a monthly $149 fee before I could even make an appointment to receive any services. Next, I was told my actual payment would be $249/month, which is $100 more than my original quote. I immediately cancelled my subscription with this dubious company (I’ve never experienced a medical company that charged fees before even the promise of services were delivered).

I started FORM in January out of total…

I started FORM in January out of total desperation. I was intrigued about having an online format only because it fits in with my busy lifestyle. I found the Form program to be the only one that I’ve stuck with for so long because of its structure and the great support I receive from my MD, my dietitian, and the support staff. My weight loss is slow and steady and it helps keep me accountable to keep moving forward when I hit those inevitable plateaus. Big thank yous! Doctors dietitians and support staff. I feel that eventually I will be successful where I have failed with so many of the other weight loss programs I’ve been on. This program is more like learning how to change my lifestyle so I can continue to be successful.

I’m so glad I decided to use Form

I’m so glad I decided to use Form for my weight loss journey. My doctor is a specialist in weight loss having years of experience before transitioning to telemedicine. She listens to me and gives me advice but lets me make my own decisions. When I send her messages, she always responds. I love the dietician support. She also has had years of experience with weight loss. I trust them both and know they are monitoring me and using reasonable weight loss strategies. I have lost 27 pounds in the last 6 months, reduced my blood pressure, and I am feeling better about myself.

Very convenient, super supportive and efficient as well as effective.

The doctors and dietitians were very helpful. They were able to work effectively with the team I had previously started my weight loss journey with. On great added benefit is that insurance started to pay for my medication, so my bill went from $500 a month to $50

I wouldn't hesitate to refer anyone who needs help to FORM

Form has helped me tremendously and I feel amazing because of the tools that were made available to me. Dr. Simpson and Amanda Downs, the RD are amazing, knowledgeable, reliable, so easy to talk to and I've been so happy with their knowledge. The Zoom appointments are easy and when I message either of them, I get a response back in a timely manner. I have lost 51 pounds since I started the program and feel terrific.

Great support team

I absolutely love my care team! I enjoy the classes, videos on social media and support group. The reason I didn’t give a 5 star review was because the billing is confusing and not self explanatory. When you try to reach billing by phone you can not get through. I emailed billing and it was basic answers. Not helpful.
