I did Form for several months. At first it was great, I started a medication and it was a big help. The dietician at first was helpful as well, gave some useful tips but didn’t require super strict adherence to calorie counts or things like that. However, it was a huge headache going back and forth with prior authorizations, refills, pharmacy stock issues, etc. I spent hours on the phone trying to get medication. Form doesn’t coordinate with pharmacies and they don’t publish their phone number, so my pharmacist couldn’t even call to try to coordinate with them. Ultimately, my insurance plan, after approving my PA finally, then did a blanket reversal and cancelled all GLP1s for everyone a couple months after I started. I can’t afford out of pocket full costs for the name brand medication. My doctor first assured me that we could look at other options but then it turns out none of them were appropriate for me. The doctor basically said if I wasn’t “focused” on my weight goals—ie willing to pay $1000 a month out of pocket for Wegovy—there wasn’t anything she could offer and recommended I “pause” my membership. She was adamant that compounded medication would not be safe and that the only acceptable form of GLP1s would be the name brands. The dietician kept giving me really generic advice and sending me links to food plans that included meat (I’m vegetarian ) and products with artificial sweeteners (I’m incredibly sensitive to those.) She told me I could look through these lists and look up the ingredients for each product, which honestly was just super stressful and time consuming. It didn’t feel personalized when so much of the advice was in the form of handouts not specific to my needs. I ended up cancelling after a couple of months of going off meds. Without medication, Form is just fairly generic diet advice and an app that harasses you to get on your scale. Oh and about that “free” scale they send you after you sign up and pay the fee: I got an EOB from my insurance company. Form billed them $200 a month for “remote physiologic monitoring.” That is why they require you to weigh 16 times a month—so they can bill your insurance for this “service.” This is on top of the copays you pay and the amount they bill insurance for your dietician and doctor visits. This seems super scammy to me, as they don’t actually look at your weights in between visits, and the dietician even said that frequent weights aren’t necessary. Weekly is fine for most people. The idea behind Form is solid, IF you don’t mind weighing yourself every other day; IF your insurance covers meds or you have the funds to pay the cost outright; IF you don’t have food allergies or dietary limitations that narrow your choices; and IF you have the time and energy to search for a pharmacy each month that has your medication in the dosage you need in stock.
Excellent especially RD support
Form is a really well structured service combining medical advice (including options to prescribe medicine) together with the dietitian advice. GLP1s were not covered by my insurance but I took them out of pocket for a few months. They were effective and helped me through a plateau. The game changer though was the RD: Jacqualine was absolutely amazing helping me understand eating habits, giving a ton of advice for specific foods, eating plans… a ton of details to help me make healthier choices. She was also my cheerleader and counselor. Overall I have lost 30lbs with Form (having lost 10lbs on my own before I joined) taking me into the healthy weight zone for the first time in 25 years.

Be careful with this company…
The provider and dietitian were great. It’s the company I have an issue with. I was assigned two care members (an RN and a dietitian) and after four months, they sent me a message saying I would be assigned new providers and I basically had to start all over again. I told them that was unacceptable and I canceled my membership. I really liked my two providers so for them to do this was not cool. Plus they didn’t even give me a valid reason. It’s not like they left. Just keep this in mind as you decide whether or not you want to use their service. Why for a relationship with providers and then have them changed it makes no sense.

I’m very happy with the results and support from Form Health and I already referred them to colleagues at work, who got impressed with my results. I would not be able to get where I am today if not with Form.
Set for Success!
I have been using the Form program since January 2024 and have lost 75 pounds so far. With the help of my wonderful medical team, especially my dietician Jessica, I have truly changed my relationship with food. My husband and I have adopted a plant forward diet; primarily vegetarian with occasional fish. Not only are we enjoying all the new recipes, but our cravings for processed foods have really decreased. The Zepbound prescription has helped me tremendously- I still enjoy my food, but it’s not a focus of my day and my portion sizes are reasonable. I now rarely snack between meals, and when I do I make smarter choices. The amount of support I receive with Form matches my needs perfectly; I’m not into group meetings although Form offers many opportunities for those if that’s your thing. The information that my team has at their fingertips that they send to me is always on point and informative. I love this program!
