Dr filled the form in wrong meant I got my licence revoked until I got an eye test

My experience was good but I took all the paperwork I.e letter from dr eye test results and a list of all my medication and the dr I saw didn’t even ask for it.

Eye test not included
"we complete an eye test with every medical"... not so. The doctor doing my medical advised I go back to my opticians to have the form completed by them, as the 1 month old prescription "was complicated". I asked for a partial refund for the opticians charge, but was refused. Not happy.
